Cancellation Policy
For appointments no showed, cancelled or rescheduled with less than 48 hours notice we charge a fee:
No Show, Cancelled or Rescheduled Initial Assessment: $150 fee applies
Appointments of up to 30 minutes: a $100 fee applies
Appointments of 30 minutes or more: a $150 fee applies
Follow Up appointments a $50 fee is applied
Physician Referrals
We value the trust and collaboration of our referring physicians. If you have a patient who could benefit from our specialized care, please fax a referral to 403-770-8941.
